How to get an Excel form to allow multiple answers and record them separately?

I am making an Excel form to record testing results. I made one last year and it worked fine, but it was clunky, this year I'd like to update it and get it working more smoothly.

The data I'm collecting are whether or not an allergen test passes. We have multiple buildings, each with multiple rooms, and then multiple pieces of equipment inside each room. When tests are needed they only happen in one room at a time, but multiple pieces of equipment will be tested.

I would like to find a way that we can start the form and answer questions about the date and which building and room, and then have a question about the equipment that allows you to select multiple answers. Then I want to be able to answer pass or fail for each selected equipment.

Theoretically I could brute force it, and make a separate question on passing or failing for each and every piece of equipment, but that would be a ton of questions and I'm not sure how well it would translate to a chart for trending purposes. Last year I just had the form only accept one answer, so to submit all the results for a room where 5 pieces of equipment were swabbed, they would have to submit the form 5 times.

I would like a more streamlined solution, if there is one.
